一、报名条件

依法取得营业执照或企业法人营业执照的生产者、销售者，可根据自身经营需要，申请注册制造商识别码。

条码应用

2.注册程序

一、报名所需材料：

（一）中国商品条码系统会员登记表（一式两份）

（2）营业执照复印件（一式两份）

（3）汇款凭证复印件（两份）

（四）组织机构代码证复印件（一份）

2、填妥的《中国商品条码系统会员登记表》加盖公司公章后提交。公司公章上的公司名称应与公司营业执照上的公司名称一致。

条码处理

说明：《中国商品条码制度会员登记表》中的“企业类别”、“经济类型代码”、“国民经济行业分类代码”、“登记行政区划代码”请参阅“商品条码分类及代码”查询手册》填写项目内容。如果您不知道如何填写，请留空。

3、集团公司请填写《集团公司子公司基本信息表》。

5、申请人的申请材料经办公厅审核后，对符合条件的材料进行签字、报批。

6、中国物品编码中心收到初审通过的申请材料和申请人缴纳的费用（见表1）后，对符合规定条件的，由中国物品编码中心批准向申请人注册。 .制造商识别码完成审批流程。

7、注册完成后，中国商品编码中心将厂商识别码通知中国商品编码中心内蒙古分公司，原片由中国商品编码中心内蒙古分中心制作。中国商品编码中心内蒙古分公司向企业颁发《中国商品条码系统会员证》及原片。

8、申请人已收到《中国商品条码系统会员证》和原片，申请结束。
